Vaccination Requirements & Policy

All students enrolled in a New York State College must comply with New York State Public Health Laws 2165 & 2167. Immunization & Vaccination Policy. 

• Students required to submit documentation
• Born on or after January 1, 1957
• Enrolled in 6 or more credit hours with on-campus class(es)
  
• Records required
• 2 Measles, 1 Mumps, 1 Rubella dose (usually given as 2 MMR vaccines) or proof of serologic immunity
• Meningitis vaccine received within the last 5 years OR 2 doses of of Men B OR Signed response form indicating you do not intend to receive the vaccine
• Active Military/Veterans: Proof of military ID/honorable discharge, if issued within the past 10 years, may be temporarily accepted until complete immunization records are received
  
• Exempt Students
• Online-only degree programs
• Remote classes only
• Students enrolled in 5 or less credit hours
• High school students participating in Bona Bridge Dual Enrollment program
  
• Medical or Religious Exemption for Measles, Mumps, Rubella
• Medical Exemption Request
• Religious Exemption Request 

• Personal Health History & Physical Examination Form - The physical examination does not need to be completed on the St. Bonaventure University form. Students may submit a copy of their physical examination completed by their healthcare provider.

Students who take ongoing or chronic prescription medications should coordinate with their prescribing provider before arriving on campus to ensure continuity of care during the academic term. This may include arranging 90-day prescription supplies or having prescriptions sent to a local pharmacy for refills while on campus. The closest pharmacy to campus is the Walmart Pharmacy in Olean.

Please note that the Center for Student Wellbeing does not prescribe or manage chronic medications. Students should work directly with their established healthcare provider to develop a plan for medication refills during the academic session.

Students are encouraged to bring a small first aid and illness kit when preparing for campus. Having basic health supplies readily available can help manage minor illnesses or injuries without needing immediate medical care.

Consider including items such as:

• Pain relievers (e.g., acetaminophen or ibuprofen)
• Cold and flu medications
• Cough drops or throat lozenges
• Thermometer
• Bandages and basic first aid supplies
• Allergy medication
• Any personal over-the-counter medications you commonly use
• List of any known allergies 

Keeping these items in your residence hall room can help you respond quickly to minor health concerns while adjusting to campus life.
