Program Evaluation
You have completed what will prove to be one of the most exciting experiences of your life. I hope that you had a successful and enjoyable academic semester.
We are asking for your help to evaluate and improve our study abroad programs. For this reason, we ask that you complete and submit the Study Abroad On-line Program Evaluation. We trust your experience was positive; yet, there may have been some aspects of your program that you may feel need to be improved upon.

You can request transcripts by downloading and completing the form below. Each transcript request requires a $5.00 charge. Please fax the form with your credit card information directly to the Registrar’s office at (716) 375-2135.
Students from SBU and CCIS member institutions please note: IT IS NOT NECESSARY FOR YOU TO FILE A TRANSCRIPT REQUEST FORM. Your grades will be reported directly to the study abroad office at your home school. We do ask, however, that you complete the evaluation above.
