Learning Disabilities
- Evaluations done by a qualified individual.
- Evaluations can be no more than three years old.
- Descriptions of testing procedures, instruments used, and tests and sub-test results reported in standard scores using adult norms.
- Comprehensive evaluations include results in areas of intelligence, reading, mathematics, spelling, written language, language processing, and cognitive processing skills. Results should address areas of concern and weaknesses as well as strengths.
- Evaluation results should include a clear diagnostic statement based on test results and personal history of the student.
- Recommended tests include: Wechsler Adult Intelligence Scale 3rd Ed., Wechsler Adult Intelligence Scale Revised, Woodcock-Johnson Psycho-Educational Battery III (WJ PEB III).

- Assessment

- 3. Information Processing
- DSM IV diagnosis
- Evidence of a history of the impairment
- Evidence of a current impairment
- Rule of alternate diagnosis
- Interpretive summary
- Rationale for recommended services
- No more than one year old
- Disability Provider Information Form completed in full
Psychological Disabilities
- Evaluation done by a qualified individual
- Must be current, within one year
- Must be comprehensive and include
- History, diagnostic interview and psychological assessment
- A specific diagnosis
- Exclusion of an alternate diagnosis
- Rationale for recommended accommodations
Medical Disabilities
- Must be current, within one year
- Disability Provider Information Form completed by qualified individual
Visual Impairments
- Medical eye report
- Must be current, within one year
- Recommendations about adaptive technology or accommodations
- Disability Provider Information Form completed by qualified individual
Hearing Impairments
- Audiogram performed by a certified audiologist
- Disability Provider Information Form completed by qualified individual
